3. Information required: I thank Gujarat Govt for finally operationalzing  GUJARAT [RIGHT OF CITIZENS TO PUBLIC SERVICES] ACT 2013 w.e.f 01-04-2016, In this connection, kindly provide following information as on date of this RTI application relating to your office:

3.1 List of services which have been covered u/s 4 of your office and time limits for delivery of each service and copy of forms prescribed  for each service to be submitted to Designated Officer.

3.2 Designations, office addresses, email IDs and official mobile numbers of following officers designated in your office:

a. Designated Officer [DO]
b. Grievance Redressal Officer [ GRO]
c. Designated Authority [DA]

3.3 Details of availability of forms prescribed under this act, digitally so that same can be downloaded by citizens. [Digital India Initiative]

3.4 Number of complaints received under this act from 01-04-2016 till date of this application.

3.5 Complaints pending as on 31-08-2016 with DO, GRO and DA and top 5 reasons on record for pendency.
